This position will be responsible for implementing and coordinating case management activities relating to catastrophic cases and chronically ill members. The Complex Case Manager will communicate with members and physicians to evaluate total needs of high risk members and provide health education for members with chronic conditions. KS and MO RN license; case management, discharge planning and/or transfer coordination experience as well as significant diversified clinical experience; CCM preferred.
